It sounds as though MacTCP may not have been initialised (?)

Would this be a situation in which CUSM might be the very first TCP/IP
application you used, immediately after establishing your dialup
connection?

Or does it occur even when CuSeeMe follows the prior successful use of
other TCP/IP applications?
